Enrollment Information and Consent
This study is open to all women who had a tubal ligation and wishes to enroll and volunteer. It is supported by women who have had tubal ligations and believe they have suffered from the procedure. This is a self selective aspect of the study. We are aware that women having no problems will not seek out to join this study. Our population is selective, however, it is this population that has not been specifically evaluated at any time. Open enrollment to the study began on
September 1, 2004 and as of today, Requirements for involvement in the study:
All women enrolled are provided with a FSH Test Kit. We request volunteers to use the FSH self test and record their results within the study. Enrollment Fee: $30.00 |
| Enrolling into the Global PTS Health Study, logging in to the Study Questionnaire, and providing content and data is brought about and executed at the full direction and will of the volunteer and constitutes consent to participate in the study. |
Volunteer Consent It is understood and agreed by the volunteer that: The Global PTS Health Study is a "real-time" study. Participation is voluntary and that of contributing user provided content/data only. Study data will be made public at all times. No compensation, payment, medical treatment, or diagnosis is provided or offered to volunteers by the CPTwomen or by the sponsors/associates of the Global PTS Health Study. The data and content provided by the CPTwomen web site and the Global PTS Health Study is provided for informational and educational purposes only. The data and content is not intended in any way to be medical advice. Information obtained from www.tubal.org, their sponsors/associates, or by participation in the Global PTS Health Study, should not be used to diagnose, treat, cure or prevent any disease. If medical advice/treatment is required, seek the advice of your personal physician. The study enrollment fee is a donation/contribution to help fund, maintain, and publicize the study. It is considered a gift, is non-refundable, and not tax deductible. The Global PTS Health Study is comprised of "user provided content" which is contributed and provided by enrolled volunteers. For this content, the CPTwomen is a distributor only. The CPTwomen respects the confidentiality of the volunteer identities. Publicly, all volunteers are known as a unique id number. Volunteers give permission to be contacted directly by the study administrators to obtain additional information or clarify information if necessary. By submitting content to the study, volunteers grant the CPTwomen license to use and distribute the content. Volunteers promise and agree to submit only content which belongs to them and will not violate the property or rights of other people or organizations. The CPTwomen will not edit or monitor user provided content. However, the CPTwomen reserves the right to remove any user provided content that comes to its attention and which it believes, in its sole discretion, is illegal or violates stated policy. Content provided at www.tubal.org, the Global PTS Health Study Outline, and the study questionnaire may include typographical errors. Corrections, changes, and updates to the web site and the Health Study format may periodically occur. The CPTwomen assumes no responsibility for consequences resulting from participation in the Global PTS Health Study, from the use of information included at www.tubal.org, from use of the information obtained at a linked Internet address, or in any respect for the content of such information, including but not limited to errors or omissions, the accuracy or reasonableness of factual study data or scientific assumptions, ownership of copyright or other intellectual property rights, and the violation of property, privacy, or personal rights of others. All material at www.tubal.org site and the Global PTS Health Study is copyrighted. Content provided by volunteers become the property of the CPTwomen and may be used in future public publications and reports. Redistribution of any portion of the study (including but not limited to the study questionnaire and public results) without the express written permission of the CPTwomen is prohibited. In the event that redistribution is granted, all redistribution must include crediting the source of the printed material. Receiving no response to a request to redistribute data or information of any compilation shall not be considered an implied agreement to the request on the part of the copyright holder.
