Step 2
Forward your complaint to your state regulatory agency.  This is very easy to do.

To forward your complaint:

1. Scroll down and locate the mailing address/fax number of your state medical regulatory agency listed below.
2. Print a copy of your complaint and send by certified mail or documented fax.

Please note: Some State Medical Regulatory boards have websites which offer electronic complaint filing or customary state forms which can be printed, completed, and sent along with your copy. Upon receipt, you will be contacted by a state medical investigator to confirm your complaint and to begin their inquiry into your reported matter.

If you do not receive a reply or need further assistance call the CPTwomen at 815 834-0987 and we will strive to assist you.

After filing your complaint with your state regulatory board, you may decide to put on file your complaint with other governing organizations. For example, The Better Business Bureau (BBB), your states attorney (which often works along with state medical boards), your state consumer affairs office (which governs business practices), etc…

If your complaint occurred in a hospital setting, or caused you to be hospitalized, the hospital should be notified that you are making a formal complaint about one of their service providers.
